Location: Osage Ambulance District Headquarters – Linn, Missouri
Position Summary
The Licensed Practical Nurse (LPN) serves as a key member of the Osage Ambulance District’s Mobile Integrated Healthcare (MIH) team, providing community-based, patient-centered healthcare to residents throughout Osage, Maries, and Gasconade Counties. This position focuses on bridging gaps between acute care, chronic disease management, and preventive health through home visits, care coordination, and collaboration with local hospitals, clinics, and behavioral health partners.
The LPN will assist the MIH Paramedic(s), Registered Nurses, and other allied health professionals in delivering high-quality care in non-emergent environments—helping patients remain safely in their homes, reducing hospital readmissions, and improving overall health outcomes.
Essential Duties and Responsibilities

Conduct patient home visits under the direction of the MIH Coordinator and in accordance with established care plans.

Perform assessments including vital signs, medication reconciliation, and health education.

Assist with chronic disease monitoring and management (e.g., diabetes, hypertension, COPD, CHF).

Collaborate with hospitals, primary care providers, and behavioral health partners to ensure continuity of care.

Provide follow-up after emergency department or hospital discharge to prevent readmissions.

Assist with patient care documentation in electronic medical record (EMR) systems and MIH tracking platforms.

Participate in health screenings, community outreach, and wellness programs.

Maintain accurate patient records and report changes in condition to supervising staff.

Support public health initiatives, safety education, and harm-reduction efforts (including naloxone distribution and connection to treatment).

Serve as a liaison between patients, families, and healthcare providers to promote engagement and self-care.

Participate in staff training, continuing education, and interagency collaboration meetings as assigned.
Qualifications
Minimum Qualifications

Current Missouri Licensed Practical Nurse (LPN) license in good standing.

Current BLS certification (AHA or equivalent).

Minimum of two years of clinical nursing experience, preferably in home health, community health, primary care, or emergency/urgent care setting.

Valid Missouri driver’s license and acceptable driving record.

Excellent communication, documentation, and interpersonal skills.

Ability to work independently in home and community settings.
Preferred Qualifications

Experience working within Mobile Integrated Healthcare, Community Paramedicine, or Home Health programs.

Familiarity with chronic disease management, behavioral health, or substance use disorder support services.

Knowledge of local community resources, social service programs, and healthcare referral networks.

Experience using electronic medical records (EMR) and case management software.
Salary and Benefits
Salary Range: $43,000 – $56,000 annually, depending on experience.
